For Patients
You may pre-order the report by calling the Schertz EMS Business Office at (210) 619-1450 between 8:00 AM and 5:00 PM.
Have the following information ready:
- Patient's name
- Patient's Date of Birth
- Patient's Social Security Number
- Date, time and location of the service
- Requestor's name and contact information
If picking up the report:
Reports can be picked up at the Schertz EMS Business Office building between 8:00 AM and 5:00 PM.
If you are the patient listed on the report, you will need to show a picture ID and complete the Patient Authorization for Disclosure of Protected Health Information form.
If you are not the patient listed on the report, you will need to show a valid "Power of Attorney" document and/or a signed Patient Authorization for Disclosure of Protected Health Information form.
If having the report mailed:
Complete the Patient Authorization for Disclosure of Protected Health Information form.
Mail the form along with a copy of your Driver License or state issued ID.
If you are not the patient listed on the report, you will also need to provide a copy of a valid "Power of Attorney" or the Death Certificate if the patient is deceased.
For law offices and records retrieval companies:

The charges for reproduction and preparation of patient records are as follows:
- $10.00 Medical Record
- $5.00 Billing Record
- $21.00 Affidavit/Questions/Notary
